Visit Electoral Registration Officer who normally sits in collectorate of respective district. You can fill FORM 6 and apply.
Another way is you can reach to any of the active member of any political party. They usualy carry electoral roll registration campaigns from time to time. They can also help you.
Go Online : Application for inclusion of name in electoral roll are available online on respective Election Office Website. You can take a print out of such forms and submit the same in person.
